This crucial role involves:Undertaking comprehensive needs assessments under the Care Act 2014 for adults requiring social care support.Developing person-centred care and support plans in collaboration with individuals, their families, and carers, promoting independence and well-being.Conducting Mental Capacity Act (MCA) assessments and acting in individuals'' best interests where appropriate.Working effectively within a multi-disciplinary team, including health professionals, housing, and other community services.Managing a diverse caseload with varying levels of complexity, prioritising effectively and managing time efficiently.Safeguarding vulnerable adults and responding to safeguarding concerns in line with local procedures and national guidance.Providing advice and information to service users and their families on available resources and support options.About You:You will be a Qualified Social Worker with:Demonstrable experience in adult social care, particularly with a focus on Care Act assessments.In-depth knowledge of the Care Act 2014, Mental Capacity Act 2005, and other relevant adult social care legislation.Excellent ass
