As a social worker in Adult Social Care, youll play a crucial role in assessing and addressing the diverse needs of our community, navigating the varied challenges our vibrant city provides. Your responsibilities will also include: Conducting assessments and developing tailored care plans for service users and carers, ensuring their unique needs and priorities are addressed. Advocating choice and control, youll empower individuals to make decisions and manage risks, ensuring services are organised in alignment with their preferences and needs. Upholding professional standards continuously as our service evolves, youll monitor changing needs, develop support plans, utilise resources, and ensure Social Care remains future-ready through creative use of assistive tech.
