Our Respiratory medicine team is seeking experienced Clinicians to lead our Occupational Therapy services and develop an integrated approach to hospital discharge, aligned with latest national initiatives. Occupational Therapists play a key role in supporting patients with frailty and complex needs, aiding recovery from acute illness, and facilitating a ''home first'' strategy. We aim to enhance this by supporting transitions home through innovative pathways, improving outcomes alongside community colleagues.As a Band 7 Occupational Therapist, you will provide advanced clinical skills in assessing and managing patients with complex health and social needs, relating these to occupational therapy requirements both on the ward and at home. Your responsibilities include managing your caseload, supporting team members, and offering specialist advice to the MDT for patients outside your direct care.You will be a specialist practitioner in the assessment and management of older adult patients admitted to West Middlesex Hospital, using expert knowledge and clinical reasoning to support patients and staff across Respiratory, Cardiology, Surgical, Enhanced Care, and ICU settings, depending on your experience. You will manage a caseload with highly complex discharge planning throughout their hospital stay.Additional responsibilities include operational management and clinical leadership of the Respiratory Medicine Occupational Therapy team, service development in Critical Care, and leading the development of OT services in the ICU. You will attend relevant training and meetings to build service pathways, taking professional and legal responsibility for all aspects of your practice.You will determine patients'' understanding of occupational therapy interventions to obtain informed consent and work within legal frameworks, including with patients who lack capacity. Using in-depth clinical reasoning and a broad knowledge of treatment options, you will plan and implement tailored occupational therapy programs, monitor their effectiveness, and modify treatments as needed.Providing advice, teaching, and instructions to relatives, carers, and professionals is essential, ensuring a consistent approach to patient care. You will demonstrate safe moving and handling skills, develop discharge plans collaboratively, and have good knowledge of community services and pathways.You will ensure timely referrals to other health professionals/services, maintain comprehensive documentation, and participate in multidisciplinary meetings. Responsibility also includes managing therapy aids and equipment, conducting environmental assessments, supporting community visits, and developing the OT service in ICU to meet guidelines.Weekend occupational therapy provision may be required, including participation in the Emergency Department weekend rota. You will support the planning, development, and evaluation of the Respiratory Medicine OT service, provide leadership and peer support, assist in staff induction and training, and develop your skills in Critical Care.Professional responsibilities include adherence to HCPC standards, engaging in CPD, reflective practice, and maintaining accurate records. Effective communication with patients, carers, and the MDT is vital, including using various communication tools and ensuring legal and professional documentation is accurate.We are part of Chelsea and Westminster Hospital NHS Foundation Trust, a top-performing and safe trust serving a diverse population.
