Supporting people across the seven West London boroughs, you ll provide tailored, person-centred support that empowers people to live more independently.Key Deliverables include:Interview and assess clients in order to determine suitability for the service.Following initial assessment to go through a programme of work with clients accepted to cover accommodation needs, independent living skills, support with work and learning and social and emotional issues, then subsequent referrals to relevant agencies.Where clients have moved in to accommodation, the worker will provide support through a programme of regular visits.
