We are seeking a proactive professional to deliver person-centred, recovery-focused support across multiple Kingston properties. The ideal candidate will assess referrals, develop SMART support and risk plans, and help service users sustain tenancies, avoid relapse, and build independence. Strong partnership working with external agencies and housing providers is essential, alongside experience in housing management, health access, and promoting social inclusion. The role requires excellent record-keeping, flexibility to meet service coverage needs, and a sound understanding of relevant legislation.
