This will be on a permanent basis paying 48,618- 50,646 per annum. Responsibilities include: Carrying out assessment''s for Older Persons and their carersObtaining multi-disciplinary assessment of needCare managementDesigning intensive care packagesCarrying a caseload of service usersMake recommendations for short-term care, intermittent care and permanent residential care where appropriateSelf-directed support / personal budgetsSupervise Social Workers and Case Management OfficersHandle complex cases, including MCA Assessments, Court of Protection applications, and s42 safeguarding enquiries Experience preferred: Post qualifying experience of working in a Senior Social Worker - Older People position within the UK Social Care industry.
