We are looking for an Adult Social Worker to join the Hospital Discharge Team.This role requires a Social Work Qualification with a minimum of 2 years equivalent post qualified experience.About the Team:The ARCH (active, recovery, community, home) Team works within the hospital setting to support the flow of discharges. You will be completing Care Act assessments, mental capacity assessments and safeguarding work.
