We are seeking a Rotational Senior Clinical Pharmacist to provide expert support across a range of community hospital sites. About the Role Deliver clinical pharmacy input within community hospital and inpatient frailty services. Review and optimise medicines for patients living with frailty and polypharmacy. Support discharge planning and safe transitions of care. Contribute to service development, assisting senior colleagues with guidelines, policies, and procedures to ensure safe and effective use of medicines. Gain valuable experience through wider rotations, developing your clinical, leadership, and managerial skills. We are looking for GPhC-registered pharmacists with: Strong clinical knowledge and experience, ideally within frailty, older people, or community hospital services. Excellent communication and interpersonal skills, with the ability to work collaboratively within an MDT. A proactive and flexible approach to service delivery.
