Waltham Forest Federated GP Network is supporting the Primary Care Networks across the borough to recruit Clinical Pharmacists. The Clinical Pharmacist will play a key role in supporting the delivery of the new Network Contract DES Service specifications. This includes implementing Structured Medications Review and Optimisation requirements, addressing over-medication, inappropriate antibiotic use, and withdrawing unnecessary medicines through NHS-led programmes such as low priority prescribing and medicines optimisation. Residents in Enhanced Health in Care Homes will benefit from regular clinical-pharmacy-led medicines reviews.Main duties of the job

Work as part of a multi-disciplinary team in a patient-facing role to assess and treat patients using their expertise in medicines for specific disease areas.Hold a prescribing qualification and collaborate with the multi-disciplinary team across the Primary Care Network.Manage the care of patients with chronic diseases, undertaking clinical medication reviews to proactively manage those with complex poly-pharmacy, especially the elderly, care home residents, individuals with multiple long-term conditions (notably COPD and asthma), and those with learning disabilities or autism (via STOMP - Stop Over Medication Programme).WF GP FedNet is a not-for-profit federation of 36 GP Practices in Waltham Forest. As a private limited company, we provide NHS services based in primary care, pooling the skills and resources of local GPs to deliver large-scale services aligned with the local NHS Strategy.
