Overview4 session GP. We are a friendly GP Practice that believes in strong primary care for our community, with a focus on resilience and portfolio working. The fixed term position is available while restructuring and alterations to our appointment system are undertaken.

This is an exciting opportunity to join a dedicated and elite team. We are based near the Mendip Hills, offering a high quality of life with easy commutes from Bristol and good road links to Exeter, Taunton, Bath and London. Bristol International Airport is 9 miles away. There are excellent local schools in the area. A high standard of patient care, good patient access and continuity are priorities, alongside work-life balance for GPs and staff. We are supportive, continuously seeking to improve, develop and grow. We have worked to develop BRAVE (AI technology) to aid identification of individuals at risk of hospital admission to enable personalised care and support plans. We offer continuity of care for patients while maintaining clinician work-life balance.

Main duties

Provide high-quality patient care

Conduct surgery, telephone and video consultations, plus occasional home visits

Liaise with colleagues and external providers to ensure seamless care

Investigate and refer patients appropriately, in line with guidance

Review results and correspondence promptly

Provide cover for colleagues when required

Maintain excellent communication with patients and carers

Work collaboratively and effectively within the team

Manage your own workload and resources effectively

Report and follow up on significant incidents

Job Description and ResponsibilitiesThe post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients. Clinical responsibilities include:

Make themselves available to undertake surgery consultations, telephone consultations and queries, home visits, check and sign repeat prescriptions, and handle queries, paperwork and correspondence in a timely fashion

Make professional, autonomous decisions in relation to presenting problems

Assess health care needs of patients with undifferentiated and undiagnosed problems

Screen for disease risk factors and early signs of illness

Develop care plans in consultation with patients and in line with current practice disease management protocols

Provide counselling and health education

Admit or discharge patients to and from the caseload and refer to other care providers as appropriate

Record clear and contemporaneous consultation notes to agreed standards

Compile and issue computer-generated acute and repeat prescriptions and prescribe in line with the practice formulary

Undertake all normal duties and responsibilities of a GP in primary care

Contribute to evaluation/audit, clinical standard setting and the development of computer-based patient records

QualificationsEssential

Vocationally trained and accredited GP, preferably MRCGP

Current GMC registration

Excellent communication and interpersonal skills

Ability to work as part of a multi-disciplinary team and with limited supervision

IT proficiency with primary care systems (EMIS)

Excellent timekeeping and teamwork

Good understanding of GMS contracting requirements and QoF

Desirable

Experience and an interest in a specific disease area
