London Borough of Hackney ensure that their social workers have as much support as possible, helping them to keep a really good work life balance!The responsibilities of the Adults Social Worker includes:Assess the social care needs of adults in an acute hospital setting to support timely, safe dischargeComplete Care Act assessments, mental capacity assessments, and best interest decisions in line with legislationWork collaboratively with health professionals, families, and partner agencies to coordinate discharge planningArrange appropriate packages of care, placements, and community support servicesSafeguard adults at risk and respond to concerns in line with statutory duties and local proceduresContribute to multidisciplinary team meetings and discharge planning forumsMaintain accurate, timely, and legally compliant case recordsPromote strengths-based, person-centred practice and support service user choice and independenceManage a complex caseload, prioritising effectively to meet service demands and discharge targets.The ideal candidate will have:Social Work England registrationE
