As a Social Worker, you''ll be part of a close-knit, supportive team helping adults manage serious mental health challenges and access the care they need to live well. Working Style: You''ll be based in the Borough for four days a week, and working from home one day, playing a key role in serving the local community. Whether you''re working on the frontline or behind the scenes, you''ll be part of a dedicated team making a real difference to residents'' lives. You will be co-located with the health team, and spend time in the Town hall and other sites. You will bring a social work perspective to discussions and provide support and knowledge. What you''ll be doing: This is a role where you''ll make a real difference. You''ll carry out assessments under the Care Act and other key legislation, working with people and their carers to develop clear, personalised support plans - including personal budgets and crisis responses when needed. You''ll also support carers through their own assessments, making sure their needs are recognised too. You''ll play a key role in safeguarding - sometimes leading on enquiries - and always keeping detailed records to make sure information is accurate and up to date.
