Support WorkerNewport Up to 13.44 per hour Days/Nights Shift Work PermanentIntroductionAcorn by Synergie are recruiting dedicated and compassionate Support Workers to support an individual with mental health needs and a physical disability in their own home. This is a person-centred role focused on promoting independence, maintaining routines, and supporting safe engagement within the community.Key Duties:Deliver high-quality, person-centred care in line with individual support plans.Monitor mental and physical health, emotional wellbeing, and overall presentation.Support with all Activities of Daily Living (ADLs).Promote independence and a structured, meaningful daily routine.Encourage engagement in purposeful and enjoyable activities.Support access to the community, appointments, and social opportunities.Assist with faith-based routines and religious practices.Safely drive the individual''s vehicle where required.Maintain accurate and up-to-date records in line with regulatory standards.Uphold safeguarding responsibilities and maintain professional boundaries.Requirements:Registration with, or willingness to register with, Social Care Wales.Enhanced DBS check.Right to work in the UK.Full UK driving licence.Values-based approach with strong commitment to person-centred care.Strong communication skills and ability to work as part of a consistent team.Previous experience supporting individuals with mental health needs or physical disabilities (preferred).Flexibility to work a
