This role involves delivering high-quality, strengths-based assessments, care planning, and interventions for adults with complex mental health needs. You will work collaboratively with multidisciplinary partners, including NHS mental health services, to provide person-centred support that promotes recovery, independence, and safeguarding.The successful candidate will manage a varied caseload, undertake statutory duties under the Care Act, Mental Health Act, and Mental Capacity Act, and provide supervision and guidance to less experienced staff where required.
