Responsibilities

Conduct assessments to identify the needs of individuals with mental health challenges

Develop and coordinate care plans and support packages to promote independence and recovery

Support transitions between hospital, community, and other care settings

Collaborate with healthcare professionals, families, and community services

Advocate for service users to ensure their voices are heard and needs are met

Maintain accurate records and comply with statutory guidelines

Provide support in both community and hospital environments

Requirements

Social Work qualification and registration with SWE

Experience in mental health services (community, inpatient, or hospital discharge)

Strong communication, assessment, and organizational skills

Commitment to enhancing the well-being and recovery of individuals with mental health challenges

Fluency in Welsh (desirable, depending on location)
