Engage with individuals at risk, ensuring their support plans are tailor-made to meet their unique needs and aspirations. What you will do:

- Support the delivery of personalised care plans ensuring all service users are supported safely and that their individual needs and service targets are met.
- Assist clients with personal care activities, work collaboratively with other professionals involved in their care, and update care plans as necessary.
- Monitor the progress and wellbeing of service users, manage challenging behaviour in line with guidelines, and highlight changes to management promptly.
- Contribute to planning daily activities for service users, ensuring they remain motivated and have a positive experience.
- Accurately administer and record medications, ensuring protocols are always followed for the safe care of individuals.
