This role is vital in ensuring that individuals receive the care they deserve, and you will be at the heart of this mission.In this dynamic role, you will:Assess Needs: Conduct thorough needs and risk assessments in line with the Care Act 2014 and Mental Capacity Act 2005.Build Relationships: Foster strong connections with individuals and their families to understand their unique situations.Coordinate Support: Work collaboratively with social workers, occupational therapists, and other agencies to provide comprehensive care.Connect Communities: Link individuals with local services and resources to promote self-directed support.Respond to Inquiries: Address questions and concerns from clients and their families with empathy and professionalism.To excel in this role, you should have:A relevant qualification in social care or a related field.Experience in adult social care or community support.Knowledge of legislation affecting social care, including the Care Act 2014 and Mental Capacity Act 20
