Social Worker ? All Age Disability Service (0?25 Years)

We are looking for a committed and experienced Social Worker to join our dynamic All Age Disability Service, supporting children and young people aged 0-25 and their families.The Role:You''ll work within a supportive multi-disciplinary team to carry out statutory assessments and deliver high-quality care and support planning under the Care Act 2014 and Children Act 1989. This includes working with children in need, child protection plans, and transitions into adulthood.Key Responsibilities:Undertake complex casework and statutory assessmentsDevelop personalised care and support plansContribute to safeguarding investigations and attend child protection conferencesSupport young people in transition to adulthood, including preparing for independenceLiaise with education, health, and voluntary sectors to ensure coordinated supportMaintain accurate case records in line with professional and legal standardsAbout You:Qualified Social Worker with current Social Work England registrationMinimum 2 years'' post-qualifying experience (experience with 0-25 or disability services preferred)Strong working knowledge of Care Act 2014, Children Act 1989, and SEND code of practiceSkilled in building relationships with families, young people, and partner agenciesResilient, reflective, and able to manage a diverse caseloadEnhanced DBS requiredWhat We Offer:Flexible/hybrid working arrangement
