About the Role As a Homelessness Outreach Worker, you will: Provide street-based outreach to rough sleepers across the community. Build trusting and compassionate relationships with people facing homelessness. Carry out assessments and create individualised support plans based on each person s needs. Support individuals to access emergency accommodation, healthcare, benefits, substance misuse services, and long-term housing. Work closely with local authorities, housing partners, health teams, and specialist agencies. Promote safety, wellbeing, and safeguarding at all times. Keep accurate case notes, risk assessments, and monitoring data. This role is varied, rewarding, and essential in supporting people to rebuild their lives.
