We are looking for a dedicated Reablement Enabler to join our cients'' Adult Social Care team.This is a fulfilling role where you will provide intensive, short-term support to adults with diverse needs. Your work will focus on re-enabling people, helping them build on their strengths and achieve personal goals. This isn''t just about providing care; it''s about empowering individuals to do things for themselves again.What you''ll do:Deliver personalised care and support based on reablement plans.Work collaboratively with a multi-disciplinary team, including health and social care professionals.Help people with personal care, domestic tasks, and therapeutic exercises designed by therapists.Document and record progress towards reablement goals.Adhere to a strengths-based, person-centred approach.Be willing to work flexibly on a 7-day rota, including morning and evening shifts.What you''ll bring:A Level 2 NVQ or QCF Level 3 in a relevant field.Experience in domiciliary personal care within an Adult Social Care setting.An understanding of the Care Act 2014 and safeguarding principles.The ability to work independently and as part of a team.A genuine commitment to promoting independence and empowering individuals.Willingness to travel across our geographical area.If you are a positive and committed individual ready to make a real difference, we would love to hear from
