This role supports patients who have been recently discharged from hospital, enabling safe recovery and reablement within their own homes. The post holder will work collaboratively with D2A-trained provider agencies to assess patients, deliver targeted rehabilitation, and set realistic SMART goals to maximise independence and support long-term care planning. Key Responsibilities: Deliver community-based physiotherapy to adults post-discharge Carry out functional assessments within the patients home environment Set patient-led SMART goals and monitor progress toward independence Work closely with reablement care providers to facilitate seamless support Identify and recommend ongoing support needs or referrals where appropriate Contribute to timely and effective discharge planning and recovery pathways Maintain accurate documentation and communicate effectively within the MDT Essential Requi
