Main duties
Coordinate the work of healthcare professionals and non-clinical staff including volunteers involved in the care of patients registered at GP practices within the wider PCN.
Support the MDT with the weekly virtual home rounds by identifying people in need of review and by collating information for MDT reviews; provide coordination, secretarial and administrative support to the MDT.

Essential qualifications:
Computer literacy

Level 4 qualification (or relevant experience)

Essential:
Experience of administrative duties
Able to demonstrate a clear understanding of working with confidential information and an understanding of service user confidentiality
Working in a busy and demanding environment whilst delivering in a timely manner
Desirable:
xperience in use of databases

Knowledge/familiarity with medical terminology
Desirable:
Working in a multi-disciplinary setting where influence and negotiation is required
Desirable:
Experience working with healthcare professionals and/or previous experience in the NHS or social care or relevant field
