The position involves collaboration with allocated Pathway Advisors who are also part of our service team. Early engagement with young people transitioning from other services is a key aspect, and you will take the lead on completing pathway plan assessments and the initial pathway plan. Maintaining these plans in compliance with the required legislation until the young person reaches 18 years old is a crucial responsibility, at which point they fully transition to their allocated Pathway Advisor. Additionally, you will participate in our duty system, providing extra support to our young people alongside our Pathway Advisors.
