Milton Keynes is looking for an experienced Deputy Team Manager to join them in their The Reablement and Hospital Discharge Team. It will be 4 days in the office and 1 day from home.In this role you will be expected and not limited to:You will support the team manager in overseeing a busy, fast-paced team where every day is different.Implement creative solutions to help individuals maintain their independence.Collaborate with health professionals and continuing health care services.Lead the development of a supportive team environment.Provide reflective supervision and support to social workers and social work assistants.Manage and prioritize allocations based on risk, need, and urgency.Support workers in producing strengths-based assessments that accurately reflect the individual and their situation.Collaborate with health colleagues and other professionals to ensure timely discharge of individuals.Oversee the flow through all discharge services.Manage and support a team, addressing HR issues as necessary.Support colleagues within a multidisciplinary service.Solve problems and manage complex cases and risk.
