Are you an experienced Learning Disabilities Case Manager with a strong background in Continuing Healthcare (CHC)? We''re working with an NHS organisation in Yorkshire seeking a skilled interim professional to join their team for an initial 3-month contract.

This role offers hybrid working - with just 1 day a week required on site - making it ideal for someone looking for flexibility while still maintaining face-to-face engagement when needed.

• Manage a caseload of individuals with Learning Disabilities eligible for NHS Continuing Healthcare (CHC) funding

• Complete CHC assessments, reviews, and eligibility panels in line with National Framework guidelines

• Work collaboratively with multi-disciplinary teams and external providers to ensure safe and person-centred care

• Provide expert input to complex case reviews, including joint funding and placement decisions

• Maintain accurate and up-to-date records, ensuring compliance with national standards and local procedures
