As part of the transformation work being completed in Adult Social Care, linked to the diagnostic work carried out last Summer, we are looking for a Social Worker to join our Learning Disabilities and Autism Transformation team. These are small, dedicated teams of people who are interested in innovative practice and work across the County on several exciting initiatives. The role includes completing case work, ranging from the application of our strength-based approach through to taking funding requests to consistency practice meetings. There are opportunities to present and showcase your work as well as supporting localities to embed learning in their teams. Key Responsibilities: In-depth knowledge of the Care Act 2014, and other relevant legislation. Understanding of person-centred approaches and how to implement them in practice.
