Using a person-centred, strengths-based approach, you''ll:Carry out and review Needs Assessments and Care and Support PlansPromote wellbeing, re-ablement, and independenceLiaise with NHS teams, voluntary services, and community partnersApply key legislation including the Care Act 2014 and Mental Capacity Act 2005Identify eligibility for NHS Continuing Healthcare (CHC) and advocate for service usersSupport carers and promote community-based, cost-effective care solutionsWhat We''re Looking For:We''re after motivated and emotionally resilient professionals who can:Build trust with adults and carers from diverse backgroundsWork collaboratively across multidisciplinary teamsKeep clear, timely and person-centred recordsConfidently assess risk and enable choiceUnderstand the legal and ethical frameworks of adult social careQualifications and Experience:Experience in adult social care or community-based supportKnowledge of relevant legislation (Care Act, Mental Capacity Act, CHC process)Strong assessment and planning skillsSocial Work qualification not required - this role is open
