Adults Hospital Discharge Social Worker

Hampshire County Council are recruiting for a Social Worker in their Hospital Discharge Teams based out of Queen Aexander Hospital, Portsmouth

Role will be primarily duty which will include, triaging, resolving issues in a fast paced environment, safeguarding, managing expectations, speaking with patients and their NOK, managing a task list.

Responsibilities:

• Working with the Advanced Assessment Team(AAT).
• This individual will be part of a multidisciplinary "sub team" Supporting with early intervention in the hospital to reduce Length of stay.

The individual will be required to support in the following areas (not an exhaustive list):

• Completion of care act assessment and Mental capacity act assessments as needed
• Building professional MDT networks with key areas in the hospital
• Risk assessment work and close liaison with community teams around "High intensity users" and turning these individuals around
• Close links with out Short-Term Service and community bed providers to stream people rapidly to the services to avoid admissions and truncate stays.
• Use of HCC systems to provide AAT with collateral information to enable rapid decision making
• Represent AAT on wards, as required including participation on Stranded patient session if required.
