Operate in a mobile flexible service requiring duties during normal working hours and evenings, with coverage arranged as needed. Fulfill the role of a leaving care personal advisor for eligible, relevant, and former relevant young people as required by the Children (Leaving Care) Act 2000 and the Children Act 2010. This includes producing and reviewing the pathway plan and conducting regular two-monthly visits to care leavers. Assist young people in contributing to the assessment of their needs prior to their transfer to the Leaving Care Team, engaging in reviews and discussions of the pathway plan for 16/17-year-olds with CLAFH colleagues. Work within the Redbridge Leaving Care policies and procedures. Maintain appropriate statutory and administrative records, including the use of IT systems, while observing confidentiality and data protection guidelines.
