You ll work within an integrated healthcare framework to support adults (18+) with drug and alcohol dependence, delivering psychosocial interventions using harm reduction and abstinence-based approaches tailored to each individual. Please note: HMP Onley is a rural location with limited public transport options, so reliable travel arrangements are essential. What you ll be doing: Managing a caseload and coordinating recovery journeys Delivering structured 1-to-1 sessions and group programmes Completing assessments, risk assessments, and care plans Providing harm reduction advice and guidance Supporting release planning and through-the-gate continuity of care Collaborating with partners such as Healthcare, Probation, Housing, and Employment services Contributing to service performance targets and measuring outcomes Who we re looking for: Organised, confident, and able to manage a varied caseload A proactive team player who builds strong working relationships Flexible, with occasional evening/weekend shifts and travel across local projects if required Skilled in IT and maintaining accurate records Committed to safeguarding, quality, and professional standards This is a rewarding role where your wo
