In accordance with the practice timetable as agreed, the post-holderwill undertake a variety of duties, includingtelephone/video/virtual and face2face consultations, visitingpatients at home, checking and signing repeat prescriptions and dealingwith queries, paperwork and correspondence in a timely fashionIn general, the post-holderwill be expected to undertake all the normal duties and responsibilitiesassociated with a GP working within primary careAbout us

Job summary:The post-holder will manage a caseload and dealwith a wide range of health needs in a primary care setting, ensuring thehighest standards of care for all registered and temporary patients.Clinical responsibilities:In accordance with the practice timetable, as agreed, thepost-holder will make themselves available to undertake a variety ofduties, including surgery consultations, telephone consultations andqueries, visiting patients at home, checking and signing repeatprescriptions and dealing with queries, paperwork and correspondence in atimely fashionMaking professional, autonomous decisions in relation topresenting problems, whether self-referred or referred from other healthcare workers within the organisationAssessing the health care needs of patients with undifferentiatedand undiagnosed problemsScreening patients for disease risk factors and early signs ofillnessDeveloping care plans for health in consultation with patients andin line with current practice disease management protocolsProviding counselling and health educationAdmitting or discharging patients to and from the caseload andreferring to other care providers as appropriateRecording clear and contemporaneous consultation notes to agreedstandardsCompiling and issuing computer-generated acute and repeatprescriptions (avoiding hand-written prescriptions whenever possible)Prescribing in accordance with the practice prescribing formulary(or generically) whenever this is clinically appropriateIn general, the post-holder will be expected to undertake all thenormal duties and responsibilities associated with a GP working withinprimary care.Other responsibilities within the organisation:Awareness of and compliance with all relevant practicepolicies/guidelines, e.g. prescribing, confidentiality, data protection,health and safetyA commitment to life-long learning and audit to ensureevidence-based best practiceContributing to evaluation/audit and clinical standard settingwithin the organisationContributing to the development of computer-based patient recordsContributing to the summarising of patient records and read-codingpatient dataAttending training and events organised by the practice or otheragencies, where appropriate.In addition to maintaining continued educationthrough attendance at any courses and/or study days necessary to ensure thatprofessional development requirements for PREP are met, the post-holder willparticipate in any training programme implemented by the practice as part ofthis employment, with such training to include:Participation in an annual individual performance review,including taking responsibility for maintaining a record of own personaland/or professional developmentTaking responsibility for own development, learning andperformance and demonstrating skills and activities to others who areundertaking similar work.Quality:The post-holder will strive to maintain qualitywithin the practice, and will:Alert other team members to issues of quality and riskAssess own performance and take accountability for own actions,either directly or under supervisionContribute to the effectiveness of the team by reflecting on ownand team activities and making suggestions on ways to improve and enhancethe teams performanceWork effectively with individuals in other agencies to meetpatients needsEffectively manage own time, workload and resources.Communication:The post-holder should recognize the importance of effectivecommunication within the team and will strive to:Communicate effectively with other team membersCommunicate effectively with patients and carersRecognise peoples needs for alternative methods of communicationand respond accordingly.Contribution tothe implementation of services:The post-holder will:Apply practice policies, standards and guidanceDiscuss with other members of the team how the policies, standardsand guidelines will affect own workParticipate in audit where appropriate.Participate in practice meetings.Person Specification
