Dementia Adviser

As a Dementia Adviser, you will have the rewarding opportunity to provide support, information, and guidance to people with dementia and their carers; helping to maintain their independence, improving their sense of well-being, and putting them in more control of their lives. The service is unique to each service user as it is based upon their personal circumstances and support needs. In this community and home based role, you will offer support to your clients by phone, letter, email and in person home visits. You will also assist service users to access other services, providing signposting and referrals. You will work closely with the NHS Memory Assessment Service in clinics across East Lancashire and Blackburn with Darwen to offer support to people following a dementia diagnosis. You will help organise and attend dementia support hubs and other networking events. About you - Ability to support people to maintain their independence, improve their sense of well-being, and help them take more control of their lives. - Understanding of dementia and the needs of those living with dementia and their carers would be an advantage but if you have had experience supporting with other diagnosis or support requirements this would be taken into consideration. - Experience with communicating with a wide range of people, adapting your approach to ensure understanding at all levels. - Able to network effectively and collaborating with other professionals to achieve a positive outcome
