Overview

Due to a team member being seconded to a course, this post seeks an experienced, innovative clinical nurse with extensive paediatric nursing expertise to join the Continuing Care Team. The service sits within a wider multi-disciplinary service including Nursing (Specialist School Nursing, Asthma, Epilepsy, CCNT, Enuresis, Diana Palliative Care Team) and community paediatric dietetics. You will deliver an effective, person-centred service for children, young people and families with the highest level of medical needs in the borough, including technology-dependent, highly complex and/or requiring palliative care, assessing and supporting with packages of care in the community. You will have excellent organisational and interpersonal skills, leadership experience and be able to work within a senior management team to support the wider service. You should be highly motivated, caring and passionate about improving the healthcare and wellbeing of children and young people in Newham. Play a key role in managing the Children''s Continuing Care caseload within the Community Children''s Nursing Service in Newham, within the remit of the current Children and Young People\''s Continuing Care National Framework, delivering an effective and person-centred service for children, young people and their families with the highest level of medical needs in the borough, including those that are technology dependent, highly complex and/or requiring palliative care.Lead and develop the Continuing Care Team, working with the Children Community Nursing Team (CCNT), Diana Palliative Care Team and external stakeholders to implement effective service pathways and ensure delivery of key quality outcomes.Work seamlessly with other professionals and partner agencies when assessing eligibility for Continuing Care and supporting multidisciplinary teams in the assessment process; ensure the provision of care achieves individual outcomes and aligns with Personalisation and SEND agendas and supports transition to appropriate adult services.Qualifications and Experience
