You will hold a caseload and work with each young person to build confidence, develop life skills and move towards their personal goals.Key responsibilities include:Completing assessments to identify strengths, needs and goalsCreating tailored, strengths based support plansWorking with young people aged 15 and over in their homes and in the communityProviding practical support with daily living skills, budgeting and managing a tenancySupporting access to education, training, employment or volunteeringHelping young people apply for benefits and complete formsLinking with health, mental health, drug and alcohol and specialist servicesBuilding positive relationships with housing providers, social workers and partner agenciesMaintaining accurate records and providing updates at multi agency meetingsUsing a trauma informed approach in all areas of workOffering flexible support that fits around young people s ro
