EHCP Coordinator ( SEND )

EHC Coordinator Role - 307 Per day Please see responsibilities and duites below. To manage the whole assessment and placement process for Children and Young People with SEND to a high degree of quality, ensuring that assessment processes are completed within statutory deadlines.To draft proposed Education, Health and Care Plans (EHCPs) identifying key aspects of the professional advice and summarising in line with the SEN Code of Practice, legislation and Council policy.To contribute to required planning for the 14+ transition process.To prepare case briefings for SEND Panels in order to enable the decision making process.To attend meetings as a representative of SEND and Integrated Service, either in own right or in the absence of the Team Manager.To be responsible for seeking SEND placements at appropriate schools and providers and facilitate these placements.To monitor placements and attend annual and transition reviews, as appropriate ensuring that the pupil''s needs are met and the Council receives value for money. In particular, cases where placements breakdown, to take responsibility for involving other professionals and securing an alternative placement.
