This is a hybrid assessment role, offering flexibility to work from home while maintaining a collaborative office presence. You will complete Care Act assessments primarily over the phone or via virtual platforms, with occasional face-to-face visits when required. The work is fast-paced but not overly complex, requiring a confident practitioner who can manage their own caseload effectively, prioritise workloads, and deliver high-quality, person-centred assessments. Key Responsibilities Undertake proportionate assessments in line with the Care Act 2014. Provide information, advice, and guidance to adults and carers accessing services.
