Overview
Community Mental Health Meaningful Intervention PractitionerWe are looking to recruit an innovative and independent practitioner who will be provided with an opportunity to work in a developing and nurturing team. You will work in multi-disciplinary recovery-based mental health services and will be a core member of the multi-disciplinary team, participating in the provision of high quality mental health meaningful interventions. The role includes working closely with our community partners, including GP, staff and voluntary sector. The team’s vision is to move work from clinical settings into the community and focus on Newham’s population health needs to dissolve boundaries between health and community.
Responsibilities
Provide high quality and effective mental health support and care to Newham residents.Respond to the needs of adults with mental health needs and their carers in a timely manner, within statutory legislation and with regard to individual circumstances.Explore needs using a person-centred approach and effectively assess and manage risk.Plan and deliver outcome-focused meaningful interventions linked to promoting independence and social inclusion.Act as an allocated worker taking responsibility for the provision of community services to mental health service users and carers.
Qualifications
EssentialProfessional registration: Mental Health Nursing, Social Work, Occupational Therapist or Arts Therapist qualificationEvidence of continuing professional developmentPost-qualifying experience of delivering relapse prevention work and specific psychosocial interventionsDesirablePost-qualifying certificate/diploma/MSc in mental healthApproved Mental Health Professional
Experience
EssentialExperience of working in a mental health community team as a qualified member of staff implementing the recovery model of careExperience of making full psychosocial assessments and writing reportsExperience of working as part of a multi-professional teamDesirableExperience of providing support and education to carers/families and supportersExperience of working in a multi-racial inner-city area

Experience of promoting recoveryAbility to prioritise workloadAbility to work effectively as part of a community teamAbility to actively and appropriately participate in team meetingsAbility to problem solve and be innovative in daily practiceAbility to co-facilitate community group programmesDesirableAbility to teach and support junior staff and students of any disciplineAbility to participate in team continuing professional practice by facilitating themed education meetings
Knowledge
Understanding of the Mental Health Act 1983 and Mental Capacity Act 2005Understanding of the principles and practice of DIALOG+ and the Care Act 2014Understanding of key themes of clinical governance and evidence-based practice
