Locum Reablement Assessor Community-Based Service, 37 Hours Per Week, Flexible Shift Pattern We are currently seeking an experienced and motivated Locum Reablement Assessor to join a busy community-based service supporting individuals to regain independence and achieve positive outcomes following hospital discharge or within the community. This is an excellent opportunity for someone with a strong background in adult social care, reablement, or short-term support pathways who is confident working autonomously within a fast-paced multidisciplinary environment. The Role As a Reablement Assessor, you will deliver strengths-based assessments and coordinate short-term, outcome-focused interventions designed to maximise independence, recovery, and wellbeing. You will work closely with individuals, families, therapy teams, health professionals, and community partners to identify needs, develop reablement plans, manage risk, and ensure effective support is in place.
