Anexciting opportunity has arisen for a band 6 clinical nurse specialist tojoin the Regional Inherited Cardiovascular Conditions Service team based inLeeds. Experience within cardiology is essential.TheRegional ICC Service is nationally recognised and provides care for patientsand families with a wide range of inherited disorders that affect the heart.The service comprises three main elements of adult cardiology, paediatriccardiology and clinical genetics but functions as a truly multidisciplinaryteam working closely with clinical psychology, pathology, and neurology.Theservice is based within the Yorkshire Heart Centre at Leeds General Infirmarywith peripheral clinics throughout the region.Forfurther information or to arrange an informal visit please contact RachelWalker or Annabel Nixon, ICC Clinical Nurse Specialist on 07584 184602.Thisis a full time (37.5 hours) for 9 months fixed term contract initially.Applications from those wishing to work part time hours would also bewelcomed.Main duties of the job

Thispost is designed to support the ICC Consultant Cardiologists and the Band 7Clinical Nurse Specialists in the clinical care of patients and theirfamilies and streamline the assessment process. Areas of work will includeproviding an initial clinical contact for patients accessing the service,liaising with the multidisciplinary team, coordinating adult patient care, providing on going long termfollow up in clinic, supporting the cardiologists in providing in-patientcare and delivering nurse led screening clinics. They will also be involvedin supporting the band 7 nurses in establishing the sudden cardiac deathpathway.Thepost is rooted in clinical activities interacting directly with patients,relatives and clinicians. The role requires independent working withmultidisciplinary support and supervision from physicians and otherprofessionals within the team. Additional professional development isencouraged and support for external training is available. Existing degreelevel study is desirable and there will be opportunity to undertake furtherstudy as the post progresses.About us

TheJobholder is responsible for the management of a defined caseload. Thejobholder carries continuing responsibility for the assessment of care needs,the development, implementation and evaluation of programmes of care and thesetting of standards of care.Thejobholder will provide specialist nursing care, treatment and advice to aspecific patient group and will contribute to the development of thespeciality service. The jobholder will ensure cost effective use ofresources.Thejob holder will support the development of interdisciplinary and wholeservice working. As a role model theywill promote the sharing of good practice and delivery of high quality,effective and efficient care. The job holder will provide training andeducation to staff groups, patients and families/carers.TheICC service serves a population of approximately 3 million and is one of thelargest in the UK. The ICC Service in Leeds is a well-established,multidisciplinary, regional service that provides specialist care forpatients with 4 main groups of conditions:Cardiomyopathies - disorders ofheart muscleInherited arrhythmias - disordersof heart rhythmAortopathies - disorders of themajor blood vesselsNeuromuscular conditions -disorders that affect skeletal muscle as well as the heartThesuccessful applicant will support the existing team and contribute to theclinical care of patients with rare genetic diseases of the heart. They jobholder will deliver nurse-led clinical services including the assessment ofnew patients and patients under long term follow up. They will be alsoinvolved in service development and patient education. They will also support the Senior NurseSpecialists in the delivery of the Sudden Cardiac Death pathway.Person Specification

Qualifications

Registered Nurse (RN Adult for adult clinical areas or RN Child for childrens areas (Level 1 or 2)Recognised teaching/assessing qualification or equivalent experience of teaching and assessmentEvidence of continued professional developmentWilling to undergo training as necessaryDegree Level Education or evidence of working towards.Skills and Behaviours

Knowledge of current issues in healthcareSignificant and recent relevant experience working as a Band 5 Staff Nurse or equivalent post registration experience within cardiologyClinical audit and standard settingDisclosure and Barring Service Check
