Job Title: Social Worker - Older People - NottinghamshireLocation: NottinghamshirePay Rate: 30 per hourAbout the Role:Liquid Personnel is seeking for a dedicated qualified social worker to join it client''s busy older persons team in Nottinghamshire.What will your responsibilities be?In this role, you will manage a complex caseload, conducting assessments, support planning, and reviews to meet individual needs and promote independence. You will identify opportunities for Reablement services and use a Strength-Based Approach to support well-being and delay long-term care. You will contribute to Continuing Healthcare (CHC) assessments, coordinate service provision, and ensure effective outcomes. By identifying community resources and maximising individual assets, you will support preventative strategies.
