Outreach and Assessment Worker London Full-time, 37.5 hours per week on a rota As an Outreach and Assessment Worker, you''ll play a vital role in delivering front-line support through both community outreach and a dedicated assessment service. One night you could be out engaging with people in need of accommodation, and the next you might be welcoming someone into the service, developing a personalised support plan, and connecting them with housing and health resources. What you''ll do - Carry out both outreach and reception shifts as part of a rota - Build trusting relationships with individuals in need of accommodation and support them to move quickly into safe housing. - Provide trauma-informed, person-centred support, including assessments, personalised action plans and follow-on help to sustain tenancies. - Work closely with partner agencies to connect clients to health, housing, and other specialist services. - Maintain accurate records and share information securely in line with data protection standards. About you You bring experience of working with vulnerable adults, ideally within a support or social care setting, and you''re confident working late-night shifts. You can build rapport quickly, stay calm under pressure, and work well with partner agencies. Above all, you believe that everyone deserves access to safe, stable housing and the opportunity to rebuild their lives.
