Outreach and Assessment Worker London Full-time, 37.5 hours per week on a rota As an Outreach and Assessment Worker, you''ll play a vital role in delivering front-line support through both community outreach and a dedicated assessment service. One night you could be out engaging with people in need of accommodation, and the next you might be welcoming someone into the service, developing a personalised support plan, and connecting them with housing and health resources. What you''ll do - Carry out both outreach and reception shifts as part of a rota - Build trusting relationships with individuals in need of accommodation and support them to move quickly into safe housing. - Provide trauma-informed, person-centred support, including assessments, personalised action plans and follow-on help to sustain tenancies. - Work closely with partner agencies to connect clients to health, housing, and other specialist services. - Maintain accurate records and share information securely in line with data protection standards. About you You bring experience of working with vulnerable adults, ideally within a support or social care setting, and you''re confident working late-night shifts. You can build rapport quickly, stay calm under pressure, and work well with partner agencies.
