We re looking for a passionate and motivated Recovery Programme (Y Bont) Practitioner to join our award-winning team delivering Y Bont, a therapeutic 12-week abstinence-based recovery programme based on the Incentivised Substance Free Living (ISFL) wing at HMP Swansea. This is a full-time role (35 hours per week), primarily based at HMP Swansea, with occasional travel to other sites across South Wales. What You ll Be Doing Working as part of a small, dedicated team, you will: Deliver structured 1:1 and group interventions focused on substance misuse recovery Co-facilitate the Y Bont Programme across multiple cohorts each year Manage a caseload, assessments, care plans and progress reviews Use trauma-informed, harm reduction and abstinence-based approaches tailored to individual needs Work closely with prison staff, healthcare, probation, housing, employment and community partners Support release planning and continuity of care into the community Monitor outcomes, maintain accurate records and contribute to service targets Help organise
