Based in Hemel Hempstead, the scheme provides move-on accommodation within a safe, welcoming environment for adults aged 18+, where inclusion and respect are at the heart of everything we do.We take a person-centred approach, empowering people to rebuild their confidence, secure long-term housing, and move towards independent living.The RoleAssess needs and create personalised support plansPromote independent living skills such as cooking, cleaning and budgetingContribute to risk assessments and ongoing support plansSupport individuals to achieve personal goals and access opportunities, working collaboratively with colleagues and external partnersShifts:Monday - FridayAbout YouYou will have experience in care or support, communicate well with people from diverse backgrounds, and feel confident working both independently and alone, as the role involves some lone working.A full UK driving licence and access to a vehicle are required.Hightown Benefits 16,110 per annum (22.5 hours per week)/ 13.77 per hour33 days annual leave, including bank holidays, rising to 35 days with service (pro rata)Blue Light Card, access to discounts for national
